This is a translation of a JPNIC document. JPNIC provides this

1. Application for new domain name

 You may apply for a new domain name by submitting the
"Application for Registration of New Domain Name".  The application
form should be sent by E-mail or mail.  The address for E-mail is
apply@domain.nic.ad.jp.  The use of E-mail is most preferable.
If you use E-mail, please put the desired domain name in the Subject
column (sample below) of the application form and send it to the
above-mentioned address.

        Subject: New: XXXX.YY.JP

 If you send your application form by mail, enclose it in an envelop
indicating "Domain Name Application Form Enclosed" in red on its surface,
together with another stamped envelope bearing the return address,
and send it to the following address.  The enclosed stamped envelope
will be used to send a notice to you.

 Please note that your application, if sent by postal mail, needs to be
input into the computer after arrival to JPNIC and its acceptance
may therefore require several days more than when it is sent by e-mail.

2. Treatment of application

 Upon acceptance of your application, the organization, administrative
contact and technical contact described in the application form,
as well as the registered domain name, will be registered in JPNIC
database.  The names of the administrative contact and technical
contact will be registered as personal information.  JPNIC handles
will be given to them.
 JPNIC may disclose registered information.  With the registration of
domain names, JPNIC will register the domain information and the
each personal information in its database.
 If any change is made in personal information (e.g., the name of
the administrative contact) after the registration of a domain name,
it is necessary for each one concerned to update the JPNIC database.
For the method of updating, refer to "JPNIC registration form entry
method"(db-info.txt).
 It should be noted sufficiently that application cannot be accepted
if there is an omissioon or unclear entry on the application form.

 In addition, if a false description or the like is found in the
application in the process of acceptance procedure, JPNIC may make it
invalid. If the domain name applied for has already been registered,
JPNIC may cancel it.

3. Acceptance of application and notice of registration

 JPNIC will register a domain name within ten days* after accepting
the application, provided the name is not identical with any existing
domain name and its attributes are in conformity with the definitions
given by JPNIC.  JPNIC will then give notice to that effect to the
technical contact (or the application intermediary).  Appendix 2
gives a sample of the notice.
 However, the registration procedure may be delayed by such reason that
an examination for domain name attribute is needed and so forth in
JPNIC.  In this case, JPNIC will inform the technical contact to that
effect within ten days*.

 JPNIC will examine any application submitted if it deviates from the
document "Registration of JP Domain Name" (domain-name-all.txt) with
respect to "2. Definition of JP Domain Name" and/or "3. Definition
of Attributes", or if JPNIC judges it necessary to scrutinize to
decide whether to allow the domain name to be registered.  For this
purpose, JPNIC may ask the technical contact concerned to submit a
document giving reason for the selection of the domain name in
question.
 If the domain name has been registered based on the result of
examination, JPNIC will send the technical contact a notice of domain
name registration.  If the examination has found it inappropriate to
register the domain name, JPNIC will notify the technical contact to
that effect.

4. Treatment of registered domain name

 If the registered domain name is not registered to JPNIC name server
within a year after the registration, the registration of the domain name
will be canceled.
 As a rule, an organization domain name may not be changed for one year
after its registration.

5. Precautions for Writing the Application

 In filling in your application form, please refer to Appendix1.
Also, before filling in the form, read "Registration of JP Domain Name"
(domain-name-all.txt).  Below is general criteria for filling in the
application form:

-Be sure to type the application form for mailing.

-Application forms are processed mechanically except for some cases.
 Comments and any description written outside the remarks column are
 always ignored. If you want to give a comment, put it in the remarks
 column without fail.  (There have been many cases in which messages
 were written immediately above the application form.

-For each item, the appropriate description should be given after  ]
 (end bracket).  If the description cannot be contained in the line,
 continue it to the next line.  Place TAB or one or more blanks at the
 beginning of the continuation line without writing an item (such as
 [Address]) on it.

-A line ending with : (colon) is not an entry conumn.

-Do not change item name or item sequence.  As far as possible,
 use form 'Application for New Domain Name Registration', given in 9
 below, by taking it into your file for application.

- Any item indicated as "omissible" may be omitted.  In this case,
 nothing should be entered in the description column.  Any item
 with no such indication should be filled in.

- Even if different columns are to contain the same description,
 write it separately without using such expression as "ditto" or
 "same as xx column".

- Alphabet, numbers, blanks and symbols should be half size.

- The telephone and fax numbers should use the same format as that
 of the following sample.  No characters other than half size
 alphanumeric characters, hyphen, and period may be used.

     Example: 03-1234-5678
              045-123-4567 ext. 1234

- The administrative contact/technical contact already has a JPNIC
 handle assigned by JPNIC may only write the JPNIC handle and
 omit the other items.

- If you apply for JP domain name attributes in OR for communication
 services, show the service name concerned in the organization name.
 If attributes will be used for communication service for individual
 persons, give the indication "communication service for individual
 persons" in the description of the organization.

6. Explanation of each item of Application for New Domain Name Registration
   (The each item is filled in Japanese, hence no translation is
    given here.)

[申請年月日]  Enter the year, month and day on which the application
(Date)        form is sent, such as 1992/09/25.

(Organization)

[Organization]  Enter the organization name in English.

[郵便番号]  Enter the zip code of the representing department of the
(Zip Code)      organization.

[住所]  Enter the address of the representing department of the organization.
(Address)

[Address]  Enter the address of the representing department of the
        organization in English.

[組織概要]      Enter the description of the organization.  If you apply
(Description)   for an OR domain for personal communication service, write
                "communication service for individual persons".

[Description]  Enter the description of the organization in English.

運用責任者:
(Administrative Contact)
        The administrative contact of the organization that
        will use the domain name applied.  If the administrative
        contact belongs to more than one organization or has more
        than one title, write the representative or primary ones.
        The administrative contact is to assume every responsibility
        associated with the registered domain name.
         If the administrative contact already has a JPNIC handle
        assigned by JPNIC because of previous application for an
        IP address, JPNIC data base or the like, fill in the
        [JPNICハンドル]         (JPNIC handle) only.

[JPNICハンドル]  If the administrative contact already has a JPNIC
(JPNIC Handle)  handle from JPNIC, write that JPNIC handle.  If this
                item is filled in, all the subsequent items associated
                with the administrative contact will be ignored.  If the
                administrative contact has not a JPNIC handle yet, do not
                fill in this item.

[氏名]  Write the name of the administrative contact.
(Name)

[Last, First]  Write the name of the administrative contact in English.
        Place the last name first, followed by the given name with
        a comma between them.  If a middle name is used, write it
        after the first name with a blank between them.

          Example: Goto, Yukinori
                   Kawasaki, Kouji J.
                   Yokokawa, Emiko Anne

[電子メイル]  Write the E-mail address of the administrative contact.
(E-mail)      If the address has been renewed, update the JPNIC database
              information concerned without fail.

[NICハンドル]  Enter the NIC handle if the administrative contact is
(NIC Handle)   registered in InterNIC.  If unknown, this item may be omitted.

[組織名]        Write the name of the organization the administrative contact
(Organization)  belongs to.

[Organization]  Write the English name of the organization the
                administrative contact belongs to.

[郵便番号]  Write the zip code of the location of the organization
(Zip Code)  the administrative contact belongs to.

[住所]      Write the address of the organization the administrative contact
(Address)   belongs to.  It is unnecessary to write the organization/division
            name.

[Address]  Write the address of the organization the administrative
        contact belongs to in English.  It is unnecessary to write
        the organization/division name.

[部局名]    Enter the name of the division the administrative contact
(Division)  belongs to.  This does not need to include the name of the
            organization.  This item may be omitted.

[Division]  Enter the name of the division the administrative
        contact belongs to in English.  This does not need to include
        the name of the organization.  This item may be omitted.

[肩書]  Enter the title of the administrative contact.
(Title)         This item may be omitted.

[電話番号]  Enter the telephone number of the administrative contact.
(TEL Number)

[FAX番号]     Enter the fax number of the administrative contact.
(FAX Number)  If no fax is available, omit this item.

技術連絡担当者:
(Technical Contact)
        Information about the person responsible for
        responding to clerical and technical inquires in relation to
        the network to be applied for.  After a domain name has been
        registered, any inquiry about the domain name registered will
        be made to the technical contact.
         If the technical contact already has a JPNIC handle because
        of previous application for an IP address, JPNIC database or
        the like, fill in the [JPNICハンドル](JPNIC handle) only.
        When applying for a new domain name, only one technical contact
        may be entered.  To make more than one entry, it is necessary
        to make a registration to the JPNIC database and follow the
        database update procedure.

[JPNICハンドル]  If the technical contact already has a JPNIC handle
(JPNIC Handle)   from JPNIC, write that JPNIC handle.  If this item is
                 filled in, all the subsequent items associated with
                 the technical contact will be ignored.  If the technical
                 contact has not a JPNIC handle yet, do not fill in this item.

[氏名]  Write the name of the technical contact.
(Name)

[Last, First]  Write the name of the technical contact in English.
        Place the last name first, followed by the given name with
        a comma between them.  If a middle name is used, write it
        after the first name with a blank between them.

          Example: Goto, Yukinori
                   Kawasaki, Kouji J.
                   Yokokawa, Emiko Anne

[電子メイル]  Write the currently effective E-mail address of the
(E-mail)      technical contact.
               The word "effective" means that E-mail can be sent and
              received between the technical contact and JPNIC.  Do not
              use the domain name in the process of application or any
              planned E-mail address.  If no currently effective E-mail
              address is available, this item may be omitted.
               Upon an effective E-mail is established, update the JPNIC
              database information concerned without fail.

[NICハンドル]  Enter the NIC handle if the technical contact is
(NIC Handle)   registered in InterNIC.  If no NIC handle has been assigned,
               this item may be omitted.

[組織名]        Write the name of the organization the technical contact
(Organization)  belongs to.  The name is not necessarily be identical with
                the name of the organization applied for.

[郵便番号]  Write the zip code of the location of the organization
(Zip Code)  the technical contact belongs to.

[住所]     Write the address of the location of the organization the
(Address)  technical contact belongs to.  It is unnecessary to write
           the organization/division name.

[Address]  Write the address (English) of the location of the
        organization the technical contact belongs to.  It is
        unnecessary to write the organization/division name.

[部局名]    Enter the name of the division the technical contact
(Division)  belongs to.  This does not need to include the name of
            the organization.  This item may be omitted.

[Division]  Enter the name (English) of the division the technical
        contact belongs to.  This does not need to include the name
        of the organization.  This item may be omitted.

[肩書]  Enter the title of the technical contact.  This item is
(Title)         omissible.

[電話番号]    Write the telephone number for contact to the technical
(TEL Number)  contact.

[FAX番号]     Write the fax number for contact to the technical contact.
(FAX Number)  If no fax is available, omit this item.

(Application Intermediary)
        If the technical contact does not have access to E-mail,
        JPNIC recommends the use of an application intermediary who can
        use E-mail rather than sending the application form by mail.
        If this method is employed, write the following information
        about the application intermediary involved.  If no application
        intermediary is used, omit all the related items.
        Also, this column will be omitted if the E-mail address of
        the technical contact has been filled in.
