This is a translation of a JPNIC document. JPNIC provides this
translation for convenience of those who can not read Japanese. But it

        Precautions on Filling in The IP Address Application

The Japan Network Information Center (JPNIC) allocates IP
addresses (the network address part) that maintain uniqueness
internationally.
These precautions should be read carefully before filling in
the application form for the acquisition of IP addresses and
care should be taken during filling in the form so that
there are no mistakes.  Further, the application form for
the acquisition of IP addresses is provided at the end of
this document.


1. Method of Sending the Application Form

The application can be sent either by electronic mail or
ordinary mail.  However, note that no applications are being
accepted by fax.
The address for forwarding the applications by electronic
mail is-  apply@ip.nic.ad.jp
When sending the application by ordinary mail, the envelope
containing the application should be marked in red as
follows:
   "Application for Acquisition of IP Address"
In addition, the envelope should contain TWO stamped self-
addressed return envelopes and should be sent to the
following address.  These two envelopes will be used for
sending back the acknowledgment of receipt of the
application and the report of address allocation.

2. General Precautions

When sending the application by ordinary mail, make sure
that the form is filled in by typing.
The application form will be processed by machines except
for a part of it.  Any comments, etc., entered in the
columns other than the remarks column will all be ignored.
In particular, although many applicants tend to enter
messages immediately before the application, it should be
noted that all comments should be entered in the remarks
column only.
After the keywords enclosed within brackets [ ], enter the
corresponding contents in the same line.  When the entry
cannot be completed within that line, you may continue it in
the next line.  In the continuation lines, do not enter any
keyword (Address, etc.,) but enter one or more spaces or
tabs at the beginning of the line.
No entries should be made following words that are not
enclosed within brackets [ ].
Do not alter any keywords.  Also, never change the order of
keywords.  Never delete or add blank lines.  Since there may
be errors in copying the keywords by typing in, as far as
possible, use the Cut & Paste for copying them.
In the case of the items described in Section 3 below, if
the description "This item can be omitted." is present, the
entry for that item can be omitted if there is no
appropriate information to be entered.  When omitting an
entry, do not make entries such as "Omitted", "None", "Not
Applicable", "N.A.", etc., but leave the entry totally
blank.  In addition, do not omit the keyword even if there
is no entry to be made for that keyword.
Note that it is not permissible to omit the entries for the
items for which the description "This item can be omitted."
is not present below.
Further, even when the same entries are to be made in two
subsequent entries, do not make entries such as "Ditto", "-
"-", "Same as item *** above", etc., but make the same entry
again.
Note that alphabets, numerals, and spaces should be half-
width (single byte) characters.


3. Descriptions of Items in the Application Form
   (The each keyword is filled in Japanese, hence no translation
    is given here.)

[申請年月日](Application Date)
 Enter the date of filling in the application form in the format
 of 1992/1/1.  However, the sequence of processing of the
 application will be determined by the date of receipt of the
 application by JPNIC irrespective of the application date entered
 here.  (The date of receipt will be the next working day following
 the date of arrival of the application at JPNIC in the case of
 e-mail, and will be the date following the date of arrival by the
 number of days required for typing in the application in the case
 of ordinary mail.)

組織
(Organization)
This is the organization making the application.

[組織名]
(Organization Name)
 Enter the official name of the organization making the application
 without abbreviating.  When an individual department of the
 organization is making the application, enter the name of that
 department also.

[Organization]
 Enter the official name in English of the organization making
 the application without abbreviating.  When the organization has
 no official English name, enter a suitable English translation of
 the organization name under the responsibility of the administrative
 contact.

[郵便番号]
(Postal Code)
 Enter the postal code of the address of the organization.

[住所]
(Address)
 Enter the postal address of the head office of the organization
 making the application.

[Address]  Enter the address in English.

運用責任者
(Administrative Contact)
 This is the person in charge of the network of the organization
 making the application.  When this person belongs to several
 organizations or has several titles, etc., enter the ones that are
 considered to be the primary ones.

[氏名]
(Name)
 Enter the name of the administrative contact.

[Last, First]
 Enter the name in English of the administrative contact.  Enter
 the last name first and then the first name after delimiting by
 a comma.  If there is a middle name, enter it after the first name
 without delimiting by a comma.  In any case, make sure that only
 one comma is present in this entry.

[電子メイル]
(E-mail address)
 This item can be omitted.

[NICハンドル]
(NIC Handle)
 When the administrative contact has been registered with the NIC,
 enter the NIC handle here.  The NIC handle is a character string
 such as AB123 which is uniquely allocated to an individual.  It is
 possible to omit this entry if the NIC handle is not known.

[組織名]
(Organization Name)
 Enter the name of the organization to which the administrative
 contact belongs.  This name need not be identical to the name of
 the organization making the application.

[Organization]
 Enter the above name of the organization in English.

[住所]
(Address)
 Enter the address of the administrative contact.  Note that since
 the address, organization name, division name, and the person's name
 will be combined together at the time of preparing the address label
 for mail, there is no need to include the organization name or
 the division name in the address.

[Address]
 Enter the address in English.  As is mentioned above, there is no
 need to include the organization name or the division name.

[部局名]
(Division name)
 This item can be omitted.  There is no need for this division name
 to include the organization name.

[Division]  This item can be omitted.

[肩書](Title)
 This item can be omitted when there is no title.

[Title]
 This item can be omitted when there is no title.

[電話番号]
(Telephone Number)
 Enter the telephone number of administrative contact.

[FAX番号]
(FAX Number)
 This item can be omitted.

技術担当者
(Technical Contact)
 This is the person to be contacted for technical matters.
 All communications after the application is received will be
 made with this technical contact.
 Although this person can be the same as the administrative
 contact, in that case, do not make entries such as "Same
 as the administrative contact", etc., but enter each data
 fully again.
 Further, when this person belongs to several organizations
 or has several titles, etc., enter the ones that are
 considered to be the primary ones.

[Last, First]
 Enter the name in English of the technical contact.  Enter the
 last name first and then the first name after delimiting by
 a comma.  If there is a middle name, enter it after the first name
 without delimiting by a comma.  In any case, make sure that only
 one comma is present in this entry.

[電子メイル]
(E-mail address)
 Enter the currently valid e-mail address of the technical contact.
 The words "currently valid" mean that the e-mail address is the
 one that can be used for communication between the applicant and
 the Japan Network Information Center.  Never use a domain name that
 is still under application or any e-mail address that is intended to
 be used in the future.
 When an entry is present in this item, all future communications
 from JPNIC to the applicant will be sent to this e-mail address.
 When a person other than the technical contact of operations is to
 act as an intermediary for the e-mail, leave this entry blank
 and enter the e-mail address in the item for the Intermediary of
 Application explained below.
 This item should be omitted when making the application by
 ordinary mail.

[NICハンドル]
(NIC Handle)
 When the technical contact has been registered with the NIC,
 enter the NIC handle here.  The NIC handle is a character string
 such as AB123 which is uniquely allocated to an individual.  It is

(Application Intermediary)
 Enter the following items when there is a person other than the
 technical contact acting as the intermediary for the communications
 by e-mail.  Note, however, that the entries for the intermediary
 of application will be ignored if there is an entry in the e-mail
 address of the technical contact.

[氏名]
(Name)
 This item can be omitted if there is no intermediary of
 application.

[電子メイル]
(E-mail Address)
 This item can be omitted if there is no intermediary of
 application.

申請内容
(Contents of Application)

[ネットワーク省略名]
(Network Abbreviated Name)
 Enter the abbreviated name of the network making the application.
 (This name can be up to 12 ASCII characters long and should not
  contain any spaces.)
 This network abbreviated name will be the name that is registered
 within the database at JPNIC, etc.  This network abbreviated name
 can also be identical to another one that is already being used.

[ネットワーク総称名]
(Network Full Name)
 Enter the full name of the network with up to 20 ASCII characters
 which may include spaces.

[クラスB]
(Class B)
 Enter the number of addresses of Class B that are being requested.
 This item can be omitted when no Class B addresses are being
 applied for.

[クラスC]
(Class C)
 Enter the number of addresses of Class C that are being requested.
 This item can be omitted when no Class C addresses are being
 applied for.

[取得済IPアドレス]
(IP Addresses Already Acquired)
 Always make this entry if there are any IP addresses that have
 already been acquired by the organization making the application.
 (This includes even the addresses acquired by the parent organization
 if the applicant is only a department of an organization.)

[ホスト数]
(Number of Hosts)
 Enter the number of hosts currently being used, the number of hosts
 planned to be used after two years, and the number of hosts planned
 to be used after four years (see Note 1 below) in the format such
 as 100/200/400.  The currently value should be the actual value,
 while the future values should be the expected values.  Note that
 this entry is used only to know the approximate numbers and hence
 the number of significant digits need only be two (for example, 130).

[サブネット数]
(Number of Subnets)
 Enter the number of subnets currently being used, the number of
 subnets planned to be used after two years, and the number of subnets
 planned to be used after four years (see Note 2 below) in the format
 such as 2/3/5.  The currently value should be the actual value,
 while the future values should be the expected values.

[インターネット接続予定]
(Internet Connection Plans)
 When there is any plan to get IP connection to any IP Internet
 either inside or outside Japan (see Note 3 below), enter that network
 project name and the expected date of connection.

[備考]
(Remarks)
 When any information other that those entered in the above items
 are to be communicated to JPNIC, always make sure to enter such
 information in this field.  Note that all the fields other than this
 remarks field are processed by machine.

Note 1. Number of hosts
The [ホスト数](Number of Hosts) here is the total number of computers,
etc., in the network that carry out communication using the TCP/IP
protocol.  This does not refer merely to the so called "host computers"
and "mainframe computers" but includes work stations, personal computers
(with Ethernet boards installed in them), etc.  In addition, include
also the router units, printers, etc., that communicate based on
the TCP/IP protocol in this number.  However, there is no need to
include any terminal equipment or personal computers that do not
communicate using the TCP/IP protocol.

Note 2. Number of subnets
The [サブネット数](Number of Subnets) here is the total number of
segments that are separated by routers, etc.  In the case of networks
in which Ethernets or optical fibers are coupled together using router
or other equipment, each Ethernet or optical fiber will be counted as
one subnet.  However, when they are connected by repeaters or bridges,
the entire unit will be counted as one subnet.  The same applies to
networks made of all other media as well.
When an organization is physically distributed and is composed of
several departments which, however, are not interlinked, add up the
number of subnets in each of the departments to obtain this number of
subnets.

Note 3. IP Internet
IP Internet is the term used to comprehensively describe all the
networks taking part in various wide area network projects such as
WIDE, TISN, JAIN, KARRN, TRAIN, and SINET in Japan at present to which
the network projects such as JOIN, etc., are likely to be added in
the future.  Further, NSFnet in America and the different networks
in Europe that carry out IP connections (that is, connections based on
the TCP/IP protocol) also will be included in this meaning of the
term "Internet".
However, the networks such as JUNET in Japan or (the present) BITNET,
N1 Net, etc., are not included in this term because their connections
are not fundamentally based on the TCP/IP protocol.


4. On the Open Publication of Information

The Japan Network Information Center basically publishes openly
all the information related to IP addresses that are commonly shared
resources.  The information so made public are all the entries made
in the IP address application other than those made in the items of
number of hosts, number of subnets, Internet connection plans, and
remarks.
