                 APNIC IPv6アドレス割り振り申請フォーム

■本フォームの目的
  本フォームは、JPNICに対しAPNICへのIPv6アドレス申請取次ぎサービスを行うた
  めのものです。
  IPv6 のアドレス申請に先立っては、以下の文書をよく読んでください。

■記入上の注意
  ・プレーン ASCII で記入の上、電子メイルにてお送りください。
  ・#[  で始まる行を変更しないでください。
  ・テンプレートの項目名を変更しないでください。
  ・フォームの記入内容はそのままAPNICに送る申請内容となります。必ずすべ
    て英語で記入してください。また、全角スペース、全角文字は使用しないで
    ください。
  ・#[REQUESTOR TEMPLATE]# のacct-name:はJPNIC のIPアドレス管理指定事業者
    であることを示すために、[JPNIC会員情報](指定事業者情報)のa. [会員略称]
    と同じ文字列を記入してください。

Please answer the following questions that apply to your request.

1. If your organisation has published information online about its
   proposed IPv6 services, please provide a URL where APNIC view that
   information.



2. Please provide a network diagram showing your planned IPv6 network.
   In your diagram, please indicate approximate deployment dates for
   planned infrastructure and estimates of the amount of IPv6 address
   space to be assigned in each part of the network.



3. If you are requesting an initial allocation greater than the /32
   minimum allocation, then you may provide details of your IPv4 network
   here. Please provide a network diagram showing your IPv4 network,
   indicating the number of IPv4 customers that are currently subscribed
   to your services. This information is separate from the IPv6 custmomer
   network details provided in the "IPv6 Usage Template" above.



3. Do you have any additional comments to include here? (Refer to "Additional
   Information" in the help guide for things.)



Note: Acceptable formats for diagrams: ASCII, JPEG, GIF, PostScript, PDF,
      Visio, MS Word, MS PowerPoint.
